Continuing Care Physical Rehabilitation Services

Bon Secours Senior Assessment Program
Staffed by a geriatrician physician, nurse practitioner and a geriatric-certified RN nurse supervisor, this program offers comprehensive assessments for adults 65 and over. Assessments include complete physical examinations, social assessments, memory evaluations, medication reviews and functional assessments. With caregiver participation, the staff develops individualized care plans.

The Hospice program services the entire Hampton Roads region. A multidisciplinary team provides supportive care to terminally ill patients and their families, to meet physical, emotional and spiritual needs. The hospice program is JCAHO accredited and Medicare/Medicaid certified.
Bon Secours Private Care
Nurses' aides, under RN case supervision, provide one-on-one personal care assistance. The program is JCAHO accredited and Medicaid certified. Also offered through Private Care is HealthWatch, a personal emergency response system to help people live independently at home and feel safe and secure.
