Your First Visit to a Bon Secours Primary Care practice
Save a few minutes when you make your first visit to your Bon Secours Primary Care doctor by printing and completing your patient check-in form.
To give you the best possible care, we need the most up-to-date information about you.
What to bring to your first Primary Care visit:
- a photo ID (valid driver’s license or federal employee ID)
- your current insurance card
- bring prescribed medicines you’re taking with you
- make a note of any over the counter medications you use
- include herbs, vitamins and nutritional supplements you’re taking on the list of OTC medications
- for your child's first visit, bring your child’s immunization record
- each adult should present contact information for most recent doctor seen by them
Insurance and Payment Information
Bon Secours Primary Care practices accept most insurance plans, including Medicare, Medicaid, Tricare, Anthem, etc. Call ahead to your practice if you need to be sure.
Be prepared to pay your co-pay at the time of your visit.
For self-pay patients, payment is expected in full at the time of service. We accept cash, checks, VISA and MasterCard.
